Cardiovascular Risk Calculator — 10-year CV risk estimate

Cardiovascular risk is the probability of a major cardiovascular event (heart attack, stroke or CV death) within the next 10 years. How the cardiovascular risk calculator works

1. Base risk depends on age and sex. 2. Multipliers: smoking x1.6; BP 140-159 x1.3; BP >=160 x1.7; cholesterol 6.0-6.9 x1.2; cholesterol >=7.0 x1.5; diabetes (men) x1.5; diabetes (women) x2.0. 3. Categories: low <5%, moderate 5-10%, high 10-20%, very high >20%.

Example: male, 55 years, smoker, BP 145 mmHg, cholesterol 6.2 mmol/L

Base risk ~4.5%. Smoking x1.6=7.2%. BP 145 mmHg x1.3=9.4%. Cholesterol 6.2 mmol/L x1.2=11.2%. Result: ~11% — HIGH risk (10-20%).

What is cardiovascular risk?

The probability of a heart attack, stroke or cardiovascular death within the next 10 years, calculated from risk factors: age, sex, smoking, BP, cholesterol, diabetes.

What blood pressure levels increase risk?

Systolic BP >140 mmHg moderately, >160 mmHg significantly increases cardiovascular risk. ESC target: <130 mmHg systolic.

Why does sex influence cardiovascular risk?

Men have higher baseline risk. Oestrogens protect women before menopause. After menopause, risk rises rapidly toward male levels.

Total cholesterol >6.0 mmol/L moderately, >7.0 significantly increases risk. Lowering LDL by 1 mmol/L reduces CV events by ~22%.

Smoking damages vascular endothelium, promotes thrombosis. Smokers have 1.5-3x higher risk of heart attack and stroke.

Type 2 diabetes doubles risk in men and triples it in women. Chronic hyperglycaemia accelerates atherosclerosis.
